A new report has revealed that Hulk Hogan's death may have been caused by medical malpractice.

Hogan passed away on July 24 at the age of 71, having suffered from declining health following neck surgery in May of 2025. Hogan's official cause of death was ruled to be a heart attack, but it was also noted that he had a history of chronic lymphocytic leukaemia, a rare type of cancer.

The manner of Hogan's death was ruled to be natural, but TMZ has reported that the police report about the 911 response to Hogan's home on the day he died noted that the occupational therapist, who was present when Hogan stopped breathing, told officers that Hogan was the victim of medical malpractice as a surgeon "severed" his phrenic nerve during a recent operation.

Hogan's widow, Sky Daily, told TMZ that Hogan's phrenic nerve was "compromised" during a recent surgery and that an autopsy on her late husband had been done, although she did not reveal the results. Daily also confirmed Hogan has yet to be cremated.

The Clearwater Police Department noted in a statement that the investigation into Hogan's passing remains ongoing.

"We have been in communication with family members -- including his son, Nick, and daughter, Brooke. The unique nature of this case has required us to interview multiple witnesses and seek medical records from a variety of providers, and our detectives continue to do that. All of this takes time. The Hogan family has been appreciative of our efforts and our communication with them," the statement reads.

The phrenic nerve controls the diaphragm and is essential to a human's ability to breathe. The nerve begins in the neck near the C3 vertebra. TMZ reported that Hogan did not have a chest-clenching episode during his heart attack but simply stopped breathing.
